Costos de eventos adversos en la unidad de cuidados intensivos, en una clínica de tercer y cuarto nivel en Cali, primer semestre de 2018

Abstract

One of the main concerns of health institutions should be the control of risks that undermine the integrity of patients and their financial resources. Patient Safety involves the ongoing and proactive assessment of the risks associated with health care to constantly design and implement the necessary safety barriers. The occurrence of adverse events is the most common cause of prolonged hospital stays and increased hospital costs. Aware of the importance of quality in the provision of health services and the development of good clinical practices, this study will address the importance of costs through the detailed database review of adverse events in the intensive care unit during the first half of 2018 at a clinic in Cali. Its value lies in determining costs and assessing the quality of health service delivery; that in the future will be a contribution to improve and strengthen patient safety policies, as well as the quality program of the institution, which will increase the quality and satisfaction of users, as well as reduce the costs generated by inadequate health care. In the institution under study there is a clear patient safety policy addressed by the patient safety office, quantification or measurement of adverse events or incidents that are generated, but the study of the costs arising from these events is not carried out. With the development of this research, tools can be offered to adjust the processes of the institution under study in order for the institution to implement strategies aimed at strengthening the patient safety policy, which will allow at the same time to reduce adverse events and thus provide quality health services, in pursuit of the sustainable growth of the institution. The present work established that during the first half of 2018 in the intensive care unit a total of 56 reported adverse events were presented of which 16 adverse events were classified as preventable events that met the inclusion criteria, the most representative being the pressure ulcer. It was also possible to identify through the cost sheets established by the institution the resource plus costs for the attention of these events correspond to materials and inputs, above the costs of human resources and infrastructure. The 16 adverse events analyzed generated an overrun of $100,047,580 in the Clinic's intensive care unit service during the first half of 2018.
